Although we would like to help everyone, we curently only accept patients that are covered under a Worker's Compensation Insurance plan. Below we have attached our referral form, along with the documents we need for the patients in order to process the request for treatment. Patients, please give the attached referral form to your doctor to complete, or ask them to contact us at 956-683-0234 with questions. Providers, please complete the referral form and attach the necessary dcumentation. You can fax everything to our office at 956-683-0758 or email our pre-authorization specialist at fpc120668@gmail.com. After that, we will prepare everything and schedule the patient to begin treatment with us.
